If you are already enrolled as a Katy ISD Student, please complete the paperwork for Rank One. All athletes in Katy ISD are required to complete all online paperwork every school year to participate.

Rank One MUST be completed for your child to swim.

You will need to complete the following on Rank One:

• Physical (must be completed and dated by a physician after May 1, 2023)
• Medical History
• Handbook Acknowledgement Form
• Katy ISD Consent to Treat
• UIL Forms Signature Page
• Bonafide Residence
• Current Utility Bill (Gas, Electric or Water only)
• Birth Certificate (only if you are new to the system)

Katy ISD Sponsored Physical Examinations:

Physical examinations will only be given to Katy ISD student athletes participating in UIL activities grades 7-12. The UIL physical form will be the only physical form accepted.

Although KISD recommends the use of your family doctor for the physical examination, the following mass screenings are available as an economical convenience for families. KISD sponsored physical examinations will be performed by the Medical Colleges of Texas at a nominal fee of $30.

All payments will be onsite accepting cash, checks and credit card by phone.
